SSM Health is seeking a full-time Regional Clinical Medical Director to support its growing Hospice and Palliative Medicine team in Wisconsin. In this role, you will help shape the future of palliative care by leading program initiatives and delivering comprehensive, patient-centered care. Practice Info
• Supports inpatient, outpatient, and home-based community visits across Fond du Lac, Ripon, Waupun, Baraboo, Columbus, Madison, Janesville, and Monroe
• Partners collaboratively with 2 Physicians, 11 Advanced Practice Providers, and 15 dedicated support staff, including Registered Nurses, Social Workers, Chaplains, and Medical/Scheduling Office Assistants
Responsibilities
• .2 FTE Palliative Medical Director duties, .2 FTE Hospice Medical Director duties, and .6 FTE dedicated to Palliative Care Outpatient and Telemedicine Care
• Collaborating provider for Hospice & Palliative Care APPs
• Lead program initiatives
• Set quality and safety metrics
• Interdisciplinary team - Collaboratively work with Administrative Leaders, Clinical Staff, and Service Lines
• Help develop and grow the Regional Hospice & Palliative Medicine Program

Requirements
• Board-certified Hospice & Palliative Care Physician
• Minimum 5+ years of post-fellowship experience
• Ability to obtain a Wisconsin license
