Requirements FIXED Monday-Friday 8am-4:30pm Provider Type - MD/DO Specialty: Radiation Oncology FTE needed: 1.0 Daily Shift Standard 8hr or 12hr: 8hr Expected duration of assignment: 6 months 26 weeks Reason for opening: Gap in coverage Schedule /Shift hours: Monday-Friday 8am-4:30pm Type of privileges: Hospital Setting: Outpatient If Inpatient # of beds: N/A Call requirement: None Rounding admissions or none? None Patient demographics Adults/Geriatrics Average patients per day: Variable. 15 on Treatment daily/2-5 consults a week/5-10 follow-ups week Required Procedures: No procedures outside of radiation oncologist-consults simulations follow-ups in clinic operations. No surgery etc. Required clinical activity in the last 24 months case logs: Applicant must provide documentation case list of provision of clinical services 50 patients representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed. Will provider work solo? Yes Licensed Candidate: Yes Washington license or IMLC with LOQ required in order to meet our desired start date. Applicant will need to have at least applied for the Washington license via IMLC at time of pre-app Required certifications Must be from American Heart Association or American Red Cross as applicable: BLS EMR: EPIC Shift Info: Day 5x8-Hour 08:00 - 16:30
