Job Description
We are seeking a qualified Nurse Practitioner or Physician Assistant for a Family Practice role in Wilmington, DE. The position involves performing in-home health and wellness assessments for geriatric patients, including history and physical evaluations, medication reconciliation, health counseling, and basic screenings, documented via a tablet platform.
Key Responsibilities Include
• Conducting 5 to 8 assessments daily, primarily Monday through Sunday, 7:00 AM to 7:00 PM.
• Providing patient-focused care in a flexible, provider-driven schedule.
• Participating in assessments that involve health history, physical exams, and health education.
Qualifications Required
• Active, unrestricted NP or PA license in DE.
• National certification and active NPI.
• CAQH profile.
