Are you a compassionate LPN who wants to make a meaningful difference for patients and their families? Primrose Hospice is seeking a Licensed Practical Nurse to provide personalized nursing care that supports comfort, dignity, and quality of life throughout the end-of-life journey.

Working under the supervision and direction of a Registered Nurse, you will support each patient’s individualized Plan of Care and collaborate with our interdisciplinary team. This role combines skilled nursing care, symptom monitoring, and supportive communication with patients and caregivers.

What You’ll Do
• Provide skilled nursing care under RN supervision and within the LPN scope of practice.
• Monitor pain, symptoms, comfort, skin integrity, nutrition, hydration, mobility, and other patient needs; promptly report changes to the supervising RN.
• Administer prescribed medications and treatments, monitor patient responses, and communicate concerns.
• Perform nursing procedures such as wound care, catheter care, ostomy care, and specimen collection as ordered and permitted within your scope of practice.
• Reinforce patient and caregiver education about medications, treatments, symptom management, safety, and infection prevention.
• Provide compassionate emotional support while respecting each patient’s cultural, spiritual, and personal preferences.
• Complete timely, accurate documentation in the electronic medical record.
• Collaborate with the RN Case Manager, Director of Clinical Services, physicians, hospice aides, social workers, chaplains, and other team members.
• Follow patient privacy, infection prevention, medication safety, and agency procedures.
• Participate in staff meetings, required training, competency evaluations, and quality improvement activities.

What We’re Looking For

Required:
• Graduation from an approved practical nursing education program.
• Current, active, unrestricted Oregon LPN license.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and automobile insurance for field-based patient care.
• Strong communication, organizational, and clinical documentation skills.
• Ability to manage patient visits independently while maintaining communication with the supervising RN and care team.
• Basic proficiency with computers, electronic medical records, and mobile communication tools.
• A compassionate approach to supporting patients and families experiencing serious illness, grief, and loss.

Preferred:
• At least one year of nursing experience.
• Experience in hospice, palliative care, home health, long-term care, or skilled nursing.
• Experience caring for seriously ill or end-of-life patients.

Schedule and Work Environment

Full-time and part-time opportunities are available. Care is provided in patient homes, assisted living facilities, adult foster homes, skilled nursing facilities, and other community settings throughout our service area.

Regular travel between patient locations is required. Evening, weekend, holiday, and on-call coverage may be assigned based on patient needs and agency scheduling requirements.

Physical Requirements

Responsibilities include assisting with patient positioning, mobility, and transfers; sitting, standing, walking, bending, kneeling, and reaching; and lifting, pushing, pulling, or moving up to 50 pounds with assistance or appropriate equipment as needed. The role also requires following precautions for potential exposure to blood and other infectious materials.
