Job Description

Lead Advanced Practice Provider — Palliative Care

Nurse Practitioner / Physician Assistant

Location: Livingston, NJ
Setting: Inpatient – Cooperman Barnabas Medical Center, Livingston, NJ
Employment Type: Full-Time
Compensation: $160,000–$190,000/year + annual performance bonus up to $25,000

Lead and grow a palliative care APP team across New Jersey while maintaining meaningful clinical practice and protected leadership time as the team grows.

NJ Palliative Care Solutions is hiring a Lead Advanced Practice Provider — Nurse Practitioner or Physician Assistant to support and guide APPs delivering palliative care

This is a true clinical leadership role with protected leadership time — not a full staff caseload with extra management layered on top. As the team grows, the Lead’s direct patient load decreases to create real capacity for onboarding, mentorship, quality review, referral relationships, and clinical escalation support.

About NJ Palliative Care Solutions

NJ Palliative Care Solutions is on a mission to deliver palliative care without walls across New Jersey. We partner with leading health systems, including RWJBarnabas Health, as well as skilled nursing facilities and community-based providers, to bring high-quality palliative care to patients living with serious illness.

About the Role

The Lead Advanced Practice Provider will combine direct patient care with APP leadership, mentoring, quality oversight, documentation standards, clinical escalation support, and practical program-building.

This role may be mostly clinical at launch, but leadership time is expected to increase as the team grows. As the team expands, the Lead’s clinical caseload will be adjusted to allow time for APP onboarding, shadowing, 1:1 mentorship, case review, chart review, referral relationship management, and performance improvement.

The scope is visibly different from a staff APP role. The Lead helps set standards, strengthen execution, and improve the team’s quality, productivity, and consistency.

Why You’ll Want This Role

You will have the opportunity to shape clinical standards, mentor APPs, and help build a growing palliative care program across New Jersey.

This role offers protected leadership time that expands as the team grows, a direct patient load calibrated to leadership responsibilities, data-guided mentorship, academic pathways, CME support, and a collaborative interdisciplinary team.

The schedule is primarily Monday–Friday, with flexibility to swap workdays and a practical commitment to work-life balance.

What You’ll Do

Provide day-to-day leadership, mentoring, and clinical support to Nurse Practitioners and Physician Assistants.

Carry a focused clinical caseload calibrated to team size and leadership responsibilities.

Deliver palliative assessments, symptom management, goals-of-care conversations, and follow-up care.

Promote evidence-based standards, quality care, and consistent documentation across the team.

Support onboarding, shadowing, case review, quality review, and clinical escalation workflows.

Build referral relationships and represent NJPCS with [hospital partners / SNFs & community organizations].

Support performance improvement, productivity, quality, and consistency across the APP team.

For outpatient/SNF roles: support local and regional coverage planning, with mileage reimbursement and clinical/EMR support provided.

What You Bring

Active New Jersey Nurse Practitioner or Physician Assistant license.

National board certification.

DEA and CDS registration, or ability to obtain by start date.

5+ years of clinical experience in palliative care, hospice, geriatrics, oncology, internal medicine, family medicine, hospital medicine, SNF/LTC, or complex chronic care.

1–2+ years of leadership, mentoring, precepting, team lead, or supervisory experience.

Strong communication, clinical judgment, operational follow-through, and interdisciplinary collaboration.

A commitment to high-quality, patient-centered palliative care.
