Position Overview

Hospice El Paso is the oldest and largest non-profit hospice organization in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort and support to persons with a terminal illness, regardless of their ability to pay. Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones. Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains; plus admissions counselors and support staff. The majority of these services are provided at home. Hospice care can also be provided in nursing, foster, and assisted living homes; along with rehabilitation centers and skilled nursing facilities. The “Center for Compassionate Care” is the city's first stand-alone “hospice in-patient facility.” We provide acute hospice care in a quiet and comfortable setting where patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment. Responsibilities: To provide the highest standard of specialized holistic nursing care as prescribed by the physician in compliance with applicable RN licensure and agency policies and procedures to those patients admitted to Hospice El Paso Center for Compassionate Care (Day Shift).

Key Responsibilities

• Skilled Nursing Services

• Assessing, planning, implementing, and evaluating individual care from admission to discharge or death.

• Performs physical, psycho-social, and spiritual assessments at the start of care and establishes a plan of treatment in cooperation with the rest of the interdisciplinary team. Performs intermittent skilled nursing evaluations on assigned patients as needed.

• Works with Certified Nursing Assistants to ensure that the highest standard of patient care is provided and completes CNA duties as needed.

• Assesses and documents current pain levels and intervention outcomes with every contact and PRN.

• Educates patients to overcome identified knowledge deficits and documents all education provided.

• Reports changes in the patient’s condition to the DCO and physician as appropriate.

• Obtains physician orders and implements interventions and changes in treatment as appropriate, documenting all communication by the end of the shift.

• Assesses and evaluates medication levels each shift, ensuring the patient has a sufficient quantity and adjusting levels according to the patient’s condition.

• Maintains accuracy in recording, maintenance, and administration of medications in accordance with policy and practice.

• Initiates prescribed treatment and evaluates medication effectiveness, as designated within one hour of intervention.

• Makes appropriate referrals to other disciplines as needed for family/caregiver support.

• Follows the Practice Standard for Pronouncement of Death.

• Works with and communicates with interdisciplinary team members regarding admissions, transfers, live discharges, and changes to the plan of care.

• Provides appropriate documentation for admission.

• Revises changes in level of care, medication, and treatment, and informs all members of the interdisciplinary team of all changes. Ensures the care plan matches visit notes.

• Provides appropriate and accurate documentation of all skilled nursing assessments and interventions prior to the end of the assigned shift.

• Provides instruction, leadership, and direction to facility staff.

• Clears all chart deficiencies every week with HIMS.

• Transcribes orders from physicians and updates medications and records by the end of the assigned shift, ensuring all necessary team members are informed and all communication is documented.

• Prepares weekly interdisciplinary group reports, ensuring a report is available one day prior to the IDG meeting.

• Complies with agency policies and procedures, mission, and values statement.

• Provides accurate bedside reports to oncoming staff regarding the patient’s condition and changes in the plan of care, revising the CNA care plan as needed.

• Provides leadership and supervision to personnel when assigned.

• Covers extra shifts as needed.

• Responds to patient call bells promptly.
