Availability:
• Part-Time, Triage & On-call
• Thursday 5:30 pm - 12:00 am (Triage)
• Sunday 4 pm - 12:30 am (On-call visiting)

Key Responsibilities:
• Triage patient needs via phone
• Dispatch nurses and other clinicians
• Obtain MD orders and process them (medications, DME, etc.)
• Complete all assigned tasks (Routine Visits, PRN Visits, Continuous Care)
• Collaborate with the Interdisciplinary Team (Physician, Nursing, Social Service, and Chaplains) on individualized patient care plans
• Perform additional duties as assigned

Qualifications:
• Bilingual in English and Spanish (required)
• Must hold a valid LVN license
• Experience in a medical field is preferred
• Knowledge of medical terminology
• Able to work independently with minimal supervision
• Exceptional written and verbal communication skills
• Pleasant phone demeanor
