Role Description The Hospice RN Case Manager is a full-time, on-site role based in Hinesville, GA, responsible for coordinating and delivering comprehensive nursing care to hospice patients in alignment with individualized care plans. Day-to-day responsibilities include conducting thorough patient assessments, managing pain and other symptoms, administering medications and treatments, and monitoring changes in condition. The RN Case Manager collaborates closely with physicians, social workers, chaplains, and other interdisciplinary team members to ensure continuity of care, appropriate utilization of services, and timely discharge planning when needed. This role also involves educating patients and families on disease processes, care techniques, and available resources, advocating for patient preferences, and maintaining accurate, timely documentation that adheres to regulatory and agency standards.
Qualifications
• Candidates should possess strong Nursing skills, including hospice or palliative care experience, holistic patient assessment, and symptom management.
• Candidates should possess Case Management and Discharge Planning skills to coordinate care, develop and adjust care plans, and support smooth transitions between care settings.
• Candidates should possess knowledge of Medicine and Utilization Management to collaborate with providers, ensure appropriate use of services, and adhere to clinical and regulatory guidelines.
• Active Registered Nurse (RN) license in the state of Georgia is required.
• Ability to communicate clearly and compassionately with patients, families, and interdisciplinary team members, including strong listening and teaching skills.
• Reliable organization, time management, and documentation skills, with proficiency in electronic health records.
• Valid driver’s license, reliable transportation for local travel as needed, and ability to meet physical demands of bedside care.
• Commitment to culturally sensitive, non-discriminatory care and respect for diverse values, beliefs, and backgrounds.
