Key Responsibilities as a Hospice RN / RN Case Manager:
• Act as the primary contact for patients and families, guiding them through each stage of their hospice journey.
• Deliver skilled nursing care tailored to each patient’s individualized Plan of Care in their home or preferred living environment.
• Conduct initial and ongoing assessments to evaluate patient conditions and modify care as necessary.
• Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to create and refine personalized care plans.
• Provide emotional, physical, and spiritual support to both patients and their families, offering education, comfort, and bereavement guidance.
• Educate caregivers on safe and confident care techniques for their loved ones, combining practical training with emotional support.
• Communicate significant updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
• Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards.
• Contribute to quality improvement efforts and scheduling initiatives to provide consistent, compassionate, and efficient care.
• Assist patients and families in understanding various topics such as:
Medication administration
Hospice philosophy and services
Symptom and pain management
End-of-life processes and expectations

Essential Functions:
• Conduct comprehensive patient assessments, including in-person and telephonic evaluations, and educate patients and caregivers accordingly.
• Deliver skilled nursing interventions aligned with care goals.
• Develop and refresh individualized plans of care to address patient needs and identify risks.
• Coordinate care within interdisciplinary teams, consulting with physicians and nurse practitioners as needed.
• Offer education on disease progression, self-care techniques, nutritional needs, and end-of-life considerations to patients and their caregivers.
• Report any changes in patient conditions or decline in health to the appropriate healthcare provider.
• Support continuity of care by recognizing and aiding in necessary transitions to other levels of care.
• Document clinical records promptly, including assessment findings and care plans.
• Foster effective working relationships within the interdisciplinary group, involving team members when the patient's care plan requires collaborative effort.
• Participate in on-call and patient support activities as needed, providing competent and responsive care during such times.

About You:

We are looking for a dedicated individual who meets the following qualifications:
• Registered Nurse licensure

Experience:
• A minimum of one year as an RN in acute care, oncology, long-term care, hospice, or home health. Clinical internships may be considered.
• Experience in hospice or palliative care is preferred.

Certifications:
• Active nursing license in the relevant state
• CPR Certification
• Current auto insurance and valid driver's license

Specialized Skills:
• Understanding of the unique needs of terminally ill patients and their families, along with hospice care principles.
• Excellent patient assessment skills and effective communication abilities.
• Knowledgeable about community resources and nursing practices, as well as compliance with managed care principles and regulatory guidelines.

Working Conditions:
• Varied settings including patients' homes and institutional locations; may involve exposure to infections and hazardous materials.

Personal Traits:
• Compassionate and sensitive to the emotional challenges faced by individuals dealing with terminal illnesses.
• Ability to handle multiple tasks efficiently and adapt to changing circumstances.
• Strong interpersonal skills to communicate effectively across diverse socioeconomic backgrounds.

Tools and Equipment:
• Proficiency in using nursing assessment tools, computers, and other office equipment. Reliable transportation required for home visits.
