Hospice Nurse Case Manager
Department: Mozaic Hospice
Reports To: Hospice Program Director

About the Role
Mozaic Hospice is seeking a compassionate and clinically skilled Hospice Nurse Case Manager (RN) to join our team. The Hospice Nurse Case Manager provides comprehensive, patient-centered nursing care to individuals and families navigating serious illness and the end-of-life experience.
In this role, you will manage an assigned caseload, provide skilled nursing assessments and interventions, coordinate individualized plans of care, manage pain and symptoms, and collaborate closely with physicians and the hospice interdisciplinary team. You will serve as a trusted clinical point of contact for patients, families, caregivers, and care partners while helping ensure that each patient receives care consistent with their goals, preferences, and wishes.
The ideal candidate is an experienced Registered Nurse who is compassionate, organized, clinically confident, and comfortable working independently in home and community settings.
Key Responsibilities
• Manage an assigned caseload of hospice patients and coordinate care with the interdisciplinary team.
• Perform comprehensive and ongoing nursing assessments, identifying changes in physical, functional, psychosocial, emotional, spiritual, and safety needs.
• Develop, implement, evaluate, and update individualized hospice plans of care in collaboration with patients, families/caregivers, physicians, the Hospice Medical Director, and the interdisciplinary team.
• Provide skilled nursing care focused on comfort, pain management, symptom management, and quality of life.
• Monitor patient responses to medications, treatments, and interventions and communicate significant changes in condition promptly.
• Anticipate changes in patient needs and facilitate timely interventions to help prevent avoidable crises and support the patient's goals and preferred setting of care.
• Educate patients and families/caregivers regarding disease progression, medications, symptom management, comfort measures, equipment, safety, and expected changes in condition.
• Provide compassionate emotional support and guidance while maintaining appropriate professional boundaries.
• Coordinate hospice aide services and other interdisciplinary services to ensure care is consistent with the individualized plan of care.
• Develop and oversee hospice aide plans of care and complete required supervision in accordance with applicable requirements and agency policy.
• Participate actively in interdisciplinary group/team meetings and contribute to care plan review and revision.
• Maintain timely, accurate, and complete clinical documentation in accordance with professional standards, agency policy, and applicable requirements.
• Coordinate care with physicians, facilities, pharmacies, durable medical equipment providers, contracted providers, and other community partners.
• Coordinate hospice services as patients reside in or transition between home, assisted living, skilled nursing facilities, hospitals, and other care settings.
• Identify changes or concerns that may affect hospice eligibility or the patient's plan of care and communicate appropriately with the Hospice Program Director, Medical Director, and interdisciplinary team.
• Participate in admissions, intake/triage activities, information calls, and other clinical coverage as needed.
• Participate in the hospice on-call rotation and respond appropriately to urgent patient and family needs.
• Identify needs for social work, spiritual care, bereavement, volunteer services, complementary therapies, and other hospice resources and facilitate appropriate referrals.
• Support patient and family understanding of hospice philosophy and participate in goals-of-care discussions within the RN's scope of practice.
• Adjust visit frequency and patient/family contacts according to assessed needs, condition, goals of care, and the individualized plan of care.
• Participate in quality improvement, clinical education, competency activities, case review, and other initiatives that promote excellent patient care and hospice outcomes.
• Protect patient confidentiality and rights in accordance with HIPAA, applicable laws and regulations, and agency policies.
• Provide compassionate, culturally responsive, patient- and family-centered care that respects individual values, beliefs, preferences, and goals.
• Perform other duties consistent with the position, professional scope of practice, training, and the mission and goals of Mozaic Hospice.
Minimum QualificationsEducation
• Graduate of an accredited school of nursing.
Licensure & Credentials
• Current Registered Nurse (RN) license in the State of Connecticut.
• Current valid driver's license.
• Reliable ability to travel throughout the hospice service area as required.
Experience
• Hospice, palliative care, home health, home care, or related clinical experience preferred.
• Experience caring for patients with serious, complex, or life-limiting illness highly desirable.

Physical & Work Environment Requirements
The Hospice Nurse Case Manager must be able to perform the physical activities necessary to provide nursing care in home and community settings, including standing, walking, bending, reaching, and assisting with patient-care activities as appropriate.
The position requires travel between patient locations and the ability to safely function in varied home and facility environments.
The Nurse Case Manager must be able to manage multiple priorities, respond appropriately to urgent situations and changes in patient condition, communicate effectively in emotionally challenging circumstances, and maintain sound professional judgment.
Environmental conditions may include:
• Rare exposure to blood, body tissue, or bodily fluids.
• Possible exposure to high or low temperatures.
• Varied home and community environments.
