What You'll Do as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, guiding them throughout their hospice journey.
• Deliver and document skilled, hands-on nursing care based on each patient's individualized Plan of Care at their home or chosen location.
• Conduct initial and ongoing assessments to monitor patient condition and adjust care as necessary.
• Work collaboratively with an interdisciplinary team to develop and refine personalized care plans.
• Provide emotional, physical, and spiritual support to both patients and their families, offering education and comfort.
• Empower caregivers through training and emotional reassurance to provide safe care for their loved ones.
• Communicate timely updates regarding patient conditions to the care team for appropriate interventions.
• Maintain precise clinical documentation and adhere to infection control and compliance standards.
• Support quality improvement initiatives to ensure consistent, compassionate, and effective care.
• Assist patients and families with understanding important topics, such as:
• Medication administration
• Hospice philosophy and services
• Pain and symptom management
• End-of-life processes and expectations

Essential Functions:
• Conduct comprehensive patient assessments and reassessments, providing education to patients and caregivers as required.
• Deliver symptom management and nursing interventions aligned with individual care goals.
• Develop and update personalized care plans in response to identified needs and risks.
• Coordinate care with interdisciplinary teams and consult with physicians and nurse practitioners as needed.
• Educate patients and caregivers on disease processes, self-care, medications, and end-of-life care.
• Report changes in condition or decline to the appropriate healthcare provider.
• Facilitate continuity of care by recognizing and supporting transitions among various care levels.
• Document clinical records accurately, including assessments and progress notes.
• Foster effective relationships with the interdisciplinary team, involving them in the patient's care when necessary.
• Participate in on-call and patient support activities, conducting services competently and responsively.

About You:

To perform this job successfully, you must be capable of effectively fulfilling the essential duties listed. The requirements below represent the necessary knowledge, skills, and abilities.

Education:
• Registered Nurse

Experience:
• A minimum of one (1) year of experience as an RN in acute care, oncology, long-term care, hospice, or home health is required. Clinical hours or school internships may count as experience.
• Prior hospice or palliative care experience is preferred.

Licenses, Certifications, and Registration:
• Current nursing license in the state of employment
• CPR Certification
• Valid driver's license and automobile insurance

Specialized Knowledge and Skills:
• Knowledge of the specific needs of terminally ill patients and their families
• Excellent assessment and communication skills
• Familiarity with community resources and regulatory guidelines

Working Conditions:
• Work occurs in patients' homes and/or institutional settings with potential exposure to infections, communicable diseases, and hazardous materials.

Personal Traits, Qualities, and Aptitudes:
• Ability to manage sensitive life and death issues with empathy and care
• Flexibility and maturity in a dynamic work environment
• Effective communication across diverse socioeconomic backgrounds

Equipment/Tools/Work-Aids:
• Ability to operate nursing assessment tools, computers, and standard office equipment. Must have reliable transportation for work purposes.
