Flexible PRN RN Positions Available!
Your Role as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, guiding them through every aspect of their hospice journey.
• Deliver and document skilled nursing care tailored to each patient's individual Plan of Care in their home or assisted living setting.
• Conduct initial and ongoing assessments to effectively monitor patient conditions and adjust care accordingly.
• Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains, to develop personalized care plans.
• Provide emotional, physical, and spiritual support not only to patients but also to their loved ones, offering education, comfort, and bereavement guidance.
• Educate caregivers on how to safely care for their loved ones, providing practical training and emotional reassurance.
• Communicate updates on patient conditions to the care team to ensure timely and appropriate interventions.
• Maintain accurate clinical documentation and adhere to infection control and compliance standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Assist patients and families in understanding essential topics related to hospice care.
Key Responsibilities:
• Perform comprehensive patient assessments and reassessments, including in-person and telephonic evaluations, while providing education to patients and caregivers.
• Deliver skilled nursing interventions and manage symptoms aligned with the patients' goals of care.
• Develop and update individualized care plans to reflect patient-specific needs and risk factors.
• Coordinate care among interdisciplinary teams, actively consulting with physicians and nurse practitioners.
• Educate patients and caregivers about disease processes, self-care techniques, end-of-life care, medications, and nutritional needs.
• Report changes in patient condition or signs of decline to the relevant provider.
• Recognize and support hospice referrals and transitions across different levels of care.
• Document clinical records accurately and in a timely manner, including assessments, orders, and care plans.
• Engage with the interdisciplinary group when warranted by patient care plans.
• Participate in on-call and patient support activities, displaying clinical competence and responsiveness.

About You:
To succeed in this role, you should possess the following:
• Registered Nurse licensure
• A minimum of one (1) year of RN experience in acute care, oncology, long-term care, hospice, or home health. School-based internships may be considered.
• Preferred experience in hospice or palliative care
• Current nursing license in the state of employment
• CPR Certification
• Valid driver's license and automobile insurance
Specialized Knowledge and Skills:
• Understanding of the needs of terminally ill patients and their families, including knowledge of hospice care principles.
• Excellent patient assessment and communication skills.
• Familiarity with community resources and documentation standards.
• Knowledge of managed care principles and regulatory guidelines.
Work Environment:
• Work will occur in patients' homes, institutional settings, or in transit. Must be prepared for potential exposure to infections and hazardous materials.
Personal Traits:
• Empathy and tact in dealing with patients and families during critical moments.
• Flexibility to manage diverse tasks effectively.
