Your Role as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, assisting them through their hospice journey.
• Provide and record skilled nursing care tailored to each patient’s individual Plan of Care at their place of residence.
• Conduct initial and ongoing assessments to monitor patient conditions and adjust care plans accordingly.
• Work with interdisciplinary teams, including physicians and other healthcare professionals, to develop and refine personalized care strategies.
• Offer emotional and physical support to both patients and their families, providing education, comfort, and guidance.
• Instruct caregivers on safe and confident caregiving practices, offering both practical training and emotional support.
• Communicate important updates in patient conditions to the care team for timely interventions.
• Keep accurate clinical records and adhere to infection control and compliance standards.
• Support the organization’s quality improvement and scheduling efforts to ensure efficient, compassionate care.
• Assist patients and families in understanding key topics: • Medication administration
• Hospice care philosophy and services
• Symptom and pain management
• End-of-life processes and expectations

Essential Functions: Job Responsibilities
• Conduct thorough patient assessments and reassessments, provide education to patients and caregivers as needed.
• Deliver skilled nursing interventions consistent with care goals.
• Develop and update individualized care plans, including patient visit frequency to meet specific needs.
• Coordinate care with interdisciplinary teams, consulting with physicians and nurse practitioners.
• Educate patients and caregivers about disease processes, self-care techniques, end-of-life care, medications, and dietary needs.
• Report changes in condition to the appropriate provider.
• Ensure continuity of care by supporting relevant health care referrals and transitions in care levels.
• Document detailed clinical records for all patients, including assessment findings, orders, and care plans.
• Maintain effective relationships with the interdisciplinary group, involving team members as required to achieve desired outcomes.
• Participate in on-call and support activities when necessary, offering competent and responsive on-call services.

About You:
To be successful in this role, you need to perform each essential duty successfully. The following are representative of required knowledge, skill, and ability.
Education:
• Registered Nurse
Experience:
• At least one year of experience as an RN in acute care, oncology, long-term care, hospice, or home health. Internships and clinical hours may count as experience in individual cases.
• Preferably with hospice or palliative care experience.
Licenses, Certifications, and Registration:
• Valid nursing license in the state of employment
• CPR Certification
• Current auto insurance and valid driver’s license
Specialized Knowledge and Skills:
• Understanding of the needs faced by terminally ill patients and their families.
• Strong patient assessment skills and good communication abilities.
• Knowledge of community resources.
• Competence in documentation and program management, awareness of general nursing practices, and a firm grasp of managed care principles and regulatory guidelines.
Working Conditions:
• Working in patients’ homes or institutional settings, with potential exposure to infections, communicable diseases, and various hazardous materials.

Equipment/Tools/Work-Aids:
• Ability to operate nursing assessment tools, computers, and communication devices effectively, with reliable transportation for work purposes.
