In this dynamic position, you will provide essential care to hospice patients and their families, stepping in as needed at various locations based on staffing requirements and patient census. As a Hospice RN - Regional Support, You Will:
• Deliver compassionate and high-quality nursing care to hospice patients.
• Implement and manage individualized care plans tailored to each patient.
• Support and mentor interdisciplinary team members, including LPNs, CNAs, and HHAs.
• Address patients' physical, emotional, spiritual, and environmental needs with comprehensive assessments.
• Ensure meticulous documentation of all care provided and adherence to hospice regulations.
• Educate families and caregivers on care protocols, medication management, and safety practices.
• Participate in on-call rotations to provide after-hours support as required.
• Collaborate effectively with physicians, referral sources, and the overall care team.
• Assist in managing referrals, regulatory paperwork, and compliance with payer requirements.
• Travel to various Gentiva hospice locations to meet urgent care needs.
Schedule & Travel:
This position typically follows a Monday to Friday daytime schedule, with occasional on-call responsibilities. Travel is essential and may require limited overnight stays. Company fleet car options are available.
About You:
Qualifications - What You'll Bring:
• Valid and unrestricted RN license in the state of employment.
• Minimum of 1 year of RN experience in hospice, home health, acute, long-term care, oncology, or similar high-acuity settings.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
Preferred Experience (Not Required):
• Experience in hospice and end-of-life nursing.
• Skilled in conducting clinical assessments and communicating compassionately.
• Strong critical thinking, problem-solving, and adaptability skills.
• Comfortable working in various environments with shifting demands.
• Proficient in electronic health records and documentation.
• Calm and supportive demeanor in crisis situations, with the ability to engage families effectively.
