Job Description
Amedisys is seeking a dedicated medical professional to ensure the highest quality of life for hospice patients during their final stages. In this role, you will focus on comfort, dignity, and emotional support while working closely with a multidisciplinary team to develop individualized care plans. You will manage symptoms, oversee certifications, and provide comprehensive patient assessments across home and clinical settings.

Responsibilities
• Conduct comprehensive, face-to-face assessments with patients, manage symptoms, and provide palliative care.
• Oversee patient admissions, discharges, certifications, recertifications, and medication management.
• Participate in interdisciplinary team meetings to develop and update care plans for hospice patients.
• Maintain availability for acute patient clinical problems, consultations, and joint visits in the hospital, at home, or in collaboration with the GIP physician.
• Coordinate care, lead goals of care discussions, and maintain empathetic communication with patients, families, and caregivers.
• Educate patients and families on hospice philosophy and end-of-life care while maintaining accurate medical records.
• Provide on-call coverage for medical emergencies and patient referrals, collaborating to maintain a shared call schedule.
• Communicate with attending physicians to determine eligibility, supervise Nurse Practitioners per state requirements, and conduct quarterly meetings with the Hospice Medical Director.
Requirements
• Required: Medical degree (MD or DO) from an accredited medical school.
• Required: Current and unencumbered license to practice specific to that state assigned to work in.
• Required: Membership in good standing on the medical staff of at least one contracted hospital.
• Required: Current CPR certification.
• Required: Valid driver's license, reliable transportation, and agency required liability insurance to complete home visits.
• Preferred: Board certification in hospice and palliative medicine.
• Preferred: Hospice Medical Director certification.
