Job Description
Hospice & Palliative Care Physician – Lexington, KY
We are seeking a Hospice & Palliative Care Physician to provide care across home hospice, home-based palliative care, and limited inpatient coverage in the Lexington market.
Join a mission-driven hospice and palliative care organization providing comprehensive, patient-centered care. This is a collaborative, team-based role with strong NP support and a mix of in-person and virtual care.
Practice Highlights:
• Monday–Friday schedule with on-call duties as assigned
• Mix of home hospice and home palliative care
• Intermittent inpatient coverage at Lexington hospitals (backup support)
• Dedicated home-based team with NP support
• Virtual workdays available while on call
• Weekend call with additional compensation
• Competitive base salary with quarterly bonus incentives and sign-on bonus available
Note: Candidates interested in an Eastern Kentucky (EKY)–based role may also be considered.
Responsibilities:
• Serve as attending physician for hospice and palliative care patients
• Conduct home visits and occasional inpatient visits as needed
• Provide palliative care consultations to patients and care teams
• Participate in interdisciplinary care team meetings and care planning
• Perform certifications, recertifications, and complete required documentation in compliance with hospice regulations
Qualifications:
• MD/DO with active Kentucky license and DEA
• Board certified (FM, IM, Oncology, or related), Board certification in Hospice & Palliative Medicine preferred
• Fellowship in Hospice & Palliative Medicine or 2+ years post-residency palliative care experience required
