Hospice & Palliative Care NP Clinician — Sign-On Bonus

MJHS Hospice and Palliative Care is seeking a Nurse Practitioner to join a group of MDs and NPs providing palliative care consultations in nursing homes, hospitals, and community settings. The NP will focus on symptom management, goals of care discussions, and hospice eligibility reviews.

Responsibilities include performing initial and follow-up encounters, coordinating care plans, and contributing to quality initiatives while traveling within the assigned geographic area.
