Schedule: M-F; 8am - 5pm
Salary Range: $95,000 - $110,000/year

This position is for someone who is ready to step beyond traditional nursing duties and make a lasting impact on the clinical culture of our hospice services. You will lead interdisciplinary teams, enhance quality outcomes, and ensure that every patient receives the compassionate, dignified care that distinguishes Transitions Hospice.
What You'll Do
As the Hospice Nursing Director, your role encompasses the clinical integrity of our services across the region. Your responsibilities will focus on four key areas:
• Clinical Leadership
• Provide guidance to interdisciplinary teams to develop and personalize patient care plans that prioritize palliative comfort.
• Assess patient needs and authorize adjustments to Levels of Care accordingly.
• Facilitate nursing interventions and provide direct patient care when necessary.
• Ensure continuous, coordinated, and well-documented care through collaboration with the IDG.
• Quality & Outcomes
• Lead the investigation and resolution of quality issues alongside the Continuous Quality Improvement Committee.
• Drive patient quality metrics that contribute to your quarterly performance incentive.
• Evaluate and enhance care plan effectiveness to elevate the standard of care in your region.
• Education & Mentorship
• Act as a clinical mentor and educator to nurses, volunteers, caregivers, and community health partners.
• Encourage a culture of ongoing learning and professional development within your teams.
• Promote patient and family education to support continuity of care and positive end-of-life experiences.
• Operations & Team Management
• Collaborate with the Administrator to create and implement manpower and operational budgets.
• Oversee the hiring, supervision, evaluation, and coaching of hospice clinical staff.
• Foster a team environment rooted in accountability, compassion, and excellence.
• Champion Transitions Hospice's goals and mission across all organizational levels.

What You Bring
• Valid RN license in the state of employment
• Bachelor's degree in Nursing (BSN) preferred
• C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred.
• Five (5) + years of general nursing experience, preferably in Hospice, home health, medical, surgical, or critical care settings, with a focus on pain and symptom management interventions.
• Exceptional observation, assessment, nursing judgment, and communication skills.
• Strong multitasking abilities and the capacity to prioritize patient needs effectively.
• Demonstrated leadership experience in managing interdisciplinary clinical teams.
