Hospice Nurse - RN After Hours/On Call

AccentCare is seeking a compassionate Hospice Nurse RN to provide after-hours on-call support to hospice patients and families. In this field-based role, you will respond to urgent needs, perform assessments, manage pain and symptoms, and coordinate care with physicians and the interdisciplinary hospice team. You will deliver bedside care, provide emotional support and education to families, and ensure accurate, timely documentation in EMR systems. Drawing on Hanford Pharmaceuticals' culture of collaboration and innovation, you'll use evidence-based practices and advanced clinical skills to promote comfort, dignity, and quality of life for patients at home or in facility settings. This role requires independent decision-making, excellent communication, reliable transportation, and flexibility to work nights, weekends, and holidays as part of the after-hours rotation.

Responsibilities
• Provide after-hours and on-call hospice nursing visits in patients' homes or facilities
• Perform comprehensive assessments, triage urgent needs, and develop immediate care plans
• Manage pain and symptoms using evidence-based hospice and palliative protocols
• Collaborate with physicians and the interdisciplinary team to coordinate ongoing care
• Educate and support patients and families on disease progression, medications, and comfort measures
• Complete timely, accurate documentation in EMR and communicate status updates to the day team
• Participate in an after-hours, weekend, and holiday call rotation
• Ensure safe practice, infection control, and compliance with hospice regulations and policies
• Advocate for patient comfort, dignity, and end-of-life preferences
• Utilize reliable transportation to travel between patient locations within the service area
