As a Hospice Nurse Practitioner, you will be pivotal in delivering palliative care, symptom management, and supportive services tailored to the unique needs of each hospice patient, as directed by their referring physician.

Key Responsibilities:
• Conduct face-to-face visits with hospice patients as required and provide clinical findings to the Hospice Medical Director.
• Collaborate with the Hospice Medical Director, primary physician, and other healthcare professionals to ensure optimal patient care.
• Develop and coordinate individualized care plans, document clinical findings, and report on patient progress in coordination with the interdisciplinary team.
• Educate and mentor patients and families on self-care techniques as appropriate.
• Maintain comprehensive clinical notes on patient conditions, and communicate any changes or concerns to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active and unrestricted nursing license in your state of residence.
• Current certification as a Nurse Practitioner or Clinical Nurse Specialist in your practice state.
• Certification by a recognized national organization such as ANCC, NBCHPN, or AANP.
• At least 5 years of nursing experience.
• A valid driver's license with vehicle insurance, and access to reliable transportation.

Preferred Qualifications:
• Current CPR certification or willingness to obtain within 90 days of employment.
• Experience working collaboratively within an interdisciplinary team.
• Background in clinical nursing care in oncology, hospice, geriatrics, or intensive care settings.
• Palliative Care ARNP certification.

The hourly pay for this position ranges from $46.00 to $69.00, commensurate with full-time employment considering factors such as local labor markets and individual qualifications.
