As the Hospice Nurse Practitioner, you will deliver expert consultation in palliative care, symptom management, and supportive care tailored to the unique needs of our Hospice patients, as directed by the referring physician.
Key Responsibilities:
• Conduct face-to-face visits with eligible Hospice patients within designated timeframes, providing clinical findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director for guidance, keep the primary physician informed of recommended services, and collaborate with other medical professionals as necessary.
• Develop and coordinate the plan of care, meticulously documenting issues, goals, interventions, and responses from both the patient and family.
• Provide education and oversight to patients and their families regarding self-care techniques when appropriate.
• Maintain detailed clinical notes related to the patient's status, promptly reporting any changes or concerns to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active unrestricted license in your state of residence.
• Current certification as a Nurse Practitioner or Clinical Nurse Specialist in your practice state.
• Certification from a recognized national body, such as ANCC, NBCHPN, or AANP.
• A minimum of 5 years of nursing experience.
• A valid driver's license and vehicle insurance, along with access to reliable transportation.
Preferred Qualifications:
• Current CPR certification or willingness to obtain within 90 days of hire.
• Experience working collaboratively within an interdisciplinary team.
• Clinical experience in areas such as oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.
Compensation is competitive and based on a variety of factors, including local labor market conditions, education, work experience, and certifications. Beyond your hourly pay of $46.00 to $69.00 (based on full-time employment), we also offer a comprehensive benefits package, which includes incentive and recognition programs, stock purchase options, and a 401k contribution, all subject to eligibility requirements.
