Hospice Nurse Practitioner — Compassionate Palliative Care

Three Oaks Hospice, Inc. is seeking a Nurse Practitioner to join our palliative care team in Colorado. You will collaborate with physicians and other IDG members to deliver compassionate hospice services to patients and families.

The role requires graduation from an accredited nursing/Nurse Practitioner program, current RN and APN licenses, CPR, and experience in hospice or home health. Certification in hospice/palliative nursing is preferred; we value strong communication and reliable
