• Regional leader in hospice and palliative care

• Serves patients through its family of care providers: Agape Care South Carolina, Georgia Hospice Care, Hospice of the Carolina Foothills in North Carolina, and ACG Hospice in Alabama, Kansas, Louisiana, Missouri, Oklahoma, and Virginia

Responsibilities

• Provide patients with diagnosis, treatment, and care under the supervision of a physician

• Focus on providing palliative care

• Assist families along their health care journeys with love and compassion

Requirements

• A heart to serve patients and families and a passion for providing the best possible care

• Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner

• Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner

• Current prescriptive authority with full DEA privileges

• Must achieve and maintain national certification in the Nurse Practitioner specialty

• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care

• Hospice and/or Palliative background preferred

• Required: Reliable transportation

• Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
