Role Description The Hospice Nurse is a full-time, on-site role based in Elgin and Bastrop, TX, responsible for delivering compassionate end-of-life and palliative care to patients in their homes and facility settings. Day-to-day responsibilities include conducting comprehensive nursing assessments, administering medications and treatments, managing symptoms, and coordinating care plans with physicians, interdisciplinary team members, patients, and families. The Hospice Nurse provides education and emotional support to caregivers, ensures accurate and timely documentation, and adheres to regulatory and agency standards. This role also involves participating in care conferences, monitoring patient progress, and advocating for patient comfort, dignity, and quality of life.
Qualifications
• Strong clinical foundation in Nursing and Medicine relevant to hospice and palliative care.
• Demonstrated expertise in Hospice Care, including pain and symptom management and end-of-life support.
• Experience providing Home Care and Direct Patient Care in community or home-health settings.
• Current Registered Nurse (RN) license in the state of Texas
• Previous hospice or home health nursing experience highly beneficial.
• Excellent communication, active listening, and interpersonal skills with patients, families, and interdisciplinary teams.
• Ability to work independently, manage time effectively, and respond to changing patient needs.
• Valid driver’s license, reliable transportation, and ability to travel locally for patient visits.
