Hospice Nurse

Pride Health is hiring a Registered Nurse (RN) to support our client's home health and hospice services in Mesa, AZ.

Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM

Location: Mesa, AZ

Job Type: Contract

Contract Length: 14 Weeks

Start Date: ASAP

Weekly Hour Guarantee: 40 Hours

Coverage Area: 60-mile radius from agency

On-Call Schedule: 4:30 PM-8:00 AM Monday-Friday on a rotating basis

On-Call/Weekend Requirement: Rotation every other week | Possible weekend rotation

Mileage Reimbursement: Yes

Pay Range: $52-$54/hr
• Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide skilled nursing care to patients in home health and hospice settings.

• Carry a caseload of up to 15 patients.

• Perform all required visit types, including routine patient visits, admissions, and death visits.

• Complete accurate and timely clinical documentation using HCHB.

Requirements:

• License(s): Active Arizona RN License or Compact RN License.

• Years of Experience: 1+

• Skills/Specialties/Technology: Home health, hospice, HCHB, patient admissions, routine patient visits, death visits, clinical assessment, care coordination, and documentation.
