Hospice Liaison

Role Description The Hospice Liaison is a full-time, on-site role based in Clearlake, Webster and Galveston, TX, responsible for building and maintaining strong relationships with referral sources, healthcare providers, and community partners to promote Bristol Hospice services. In this role, the Hospice Liaison educates physicians, hospital staff, skilled nursing facilities, and community organizations about hospice and palliative care options, eligibility criteria, and the benefits of Bristol Hospice programs. The position includes meeting with patients, families, and caregivers to explain services, support informed decision-making, and facilitate smooth admissions and transitions to hospice care. Day-to-day tasks involve conducting outreach visits, participating in marketing and community events, tracking and reporting referral activity, and collaborating with the clinical team to ensure timely communication and coordinated care. The Hospice Liaison is expected to uphold the organization’s mission, maintain accurate documentation, and consistently represent Bristol Hospice with professionalism, empathy, and integrity.
