Job Description
MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals for hospice enrollment, and provide General Inpatient level care with specialized clinical skills. You will work within an interdisciplinary team to develop care plans and ensure compliant, compassionate hospice services.
The role requires NY RN licensure (CPR-BLS), 2+ years in Medical-Surgical or Community Health, and flexibility to meet patient needs, including weekend
