Hospice Care Planner Registered Nurse

Jobot is seeking a Hospice Care Planner RN to lead compassionate, patient-centered end-of-life care. In this role, you’ll manage a caseload of hospice patients, perform comprehensive assessments, and develop individualized care plans focused on comfort and dignity. You’ll coordinate interdisciplinary services, oversee pain and symptom management, and support families with education and counseling.
