We are seeking a dedicated and experienced:
HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER
Part-Time, 2-3 days per week
Job Role:
• Implement essential palliative care program requirements.
• Conduct comprehensive medical visits with patients, coordinating care with the Palliative Care team as necessary.
• Perform face-to-face encounters to provide re-certification information to the Medical Director/Associate Medical Director for hospice patients.
• Relay clinical findings from encounters to primary care physicians and/or supervising Medical Directors.
• Engage in effective communication with patients and families across all levels.
• Collaborate with agency personnel for optimal patient care.
Qualifications:
• Master's degree in Nursing required with current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner is essential.
• Preferred experience in hospice or palliative care nursing.
• Strong understanding of the framework and organization of hospice care programs.
• Exceptional communication and organizational skills.
