Home Health Occupational Therapist in Dover, DE

As a Home Health Occupational Therapist on a 13-week contract, your ability to motivate, assess, and customize treatments will be foundational to helping patients thrive in their own homes.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

• Full-time, 13-week contract

Qualifications:

• Active Occupational Therapist license (state of practice, in good standing)

• At least 1 year of recent Home Health OT experiencefield-based work preferred

• Experience with OASIS assessment required

• Proficiency in Home Care Home Base (HCHB) strongly preferred

• Reliable transportation, valid drivers license, and auto insurance

• Ability to work independently and manage a flexible field-based schedule

• Strong communication, clinical assessment, and organizational skills

Core Responsibilities:

• Conduct comprehensive OT evaluations and OASIS assessments as needed

• Develop and implement personalized care plans focused on ADLs, cognitive function, and home safety

• Deliver therapeutic activities, adaptive equipment training, and environmental modifications

• Equip patients and caregivers with practical strategies for safety and independence

• Recommend and facilitate access to appropriate durable medical equipment (DME)

• Track patient progress, adjust interventions, and communicate updates with the interdisciplinary team

• Complete timely documentation within Home Care Home Base (HCHB)

• Participate in collaborative case conferences, interdisciplinary meetings, and ongoing training initiatives

$2,016 - $2,132/Week
