Home Health Occupational Therapist in Camden Wyoming, DE

Make a direct impact on patient independence as a Home Health Occupational Therapist, providing essential care in a field-based setting. This contract role focuses on delivering high-quality therapy services for patients in their homes, optimizing their ability to perform activities of daily living and maintain safety in their own environment.

Schedule & Assignment Details:

• Full-time, 13-week contract

• Monday Friday, 8:00 AM 5:00 PM

• Caseload: 2532 points per week (57 patient visits per day)

• Daily travel within assigned territory (mileage reimbursement provided)

• Electronic documentation via Home Care Home Base (HCHB) and proficiency with OASIS assessments

Qualifications & Experience:

• Active Occupational Therapist (OT) license in good standing

• At least 1 year of recent Home Health OT experience

• Experience with HCHB (preferred) and OASIS (required)

• Strong clinical assessment, documentation, and functional training skills

• Excellent communication, time management, and organizational abilities

• Reliable transportation, valid drivers license, and auto insurance

• Ability to work autonomously and coordinate with a multidisciplinary team

Key Responsibilities:

• Conduct comprehensive OT evaluations and required OASIS assessments

• Develop individualized plans of care targeting ADLs, IADLs, and patient safety

• Deliver skilled interventions, therapeutic activities, and adaptive equipment training

• Educate patients and caregivers on independence, fall prevention, and home safety

• Maintain timely documentation in HCHB and communicate effectively with team members

• Participate in interdisciplinary meetings and maintain compliance with guidelines
