As a Home Care Nurse, you will be responsible for assessing patient needs, developing and implementing individualized care plans and educating patients and families on disease processes, procedures, and medications. This position allows you to work independently to provide the highest level of treatment and have the autonomy to use your knowledge, skills, and judgment to make decisions.

Qualifications:
• Valid RN/LPN License
• Current BLS Certification
• Proficient in patient care and assessments
• Strong communication skills
• Ability to develop and implement care plans
• No criminal record

Preferences:
• EARLY AM and LATE PM visits
• Home care experience
• EMR experience
