Home Health RN & LVN – PRN / Full-Time Opportunities
Location: Dallas–Fort Worth Metroplex, including North Dallas, McKinney, Frisco, Plano, Allen, Richardson, Carrollton, Irving, Arlington, Fort Worth, and surrounding areas
Job Type: PRN / Part-Time / Full-Time
Schedule: Flexible – Based on patient needs and clinician availability
Pay: Competitive compensation based on experience, visit type, and territory
About the Position
We are seeking compassionate, dependable, and clinically skilled Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to join our growing Home Health team.
Our nurses provide skilled nursing care to patients in the comfort of their homes while working closely with physicians, patients, families, therapists, aides, and the clinical management team.
This position is ideal for nurses who value one-on-one patient care, flexibility, autonomy, and building meaningful relationships with patients and families.
RN Responsibilities
• Perform comprehensive initial and ongoing nursing assessments.
• Evaluate patients' medical conditions, functional status, and care needs.
• Develop, implement, and update individualized plans of care in accordance with physician orders.
• Perform skilled nursing visits in the patient's home.
• Monitor patient progress and identify changes in condition.
• Communicate significant changes in patient status to the physician and clinical management team.
• Administer medications and treatments as ordered.
• Provide wound care and monitor wound healing.
• Perform medication reconciliation and identify medication-related concerns.
• Educate patients and caregivers regarding disease management, medications, treatments, safety, and plan of care.
• Coordinate care with physicians, therapists, LVNs, HHAs, and other members of the interdisciplinary team.
• Complete OASIS assessments and other required clinical documentation accurately and timely.
• Complete recertification, resumption of care, transfer, and discharge assessments as applicable.
• Review and follow physician orders and communicate when clarification or additional orders are needed.
• Participate in case conferences and care coordination activities.
• Identify potential risks, complications, and changes in the patient's condition.
• Provide appropriate follow-up and escalation when clinically indicated.
• Maintain compliance with Medicare, Texas, agency, and professional nursing requirements.
• Complete all documentation within required agency and regulatory timelines.
LVN Responsibilities
• Provide skilled nursing services to patients in their homes under the direction and supervision required by the patient's plan of care and applicable regulations.
• Perform nursing assessments and monitor patient condition.
• Administer medications and treatments as ordered.
• Provide wound care and other ordered skilled nursing treatments.
• Monitor vital signs and report significant changes in patient condition.
• Reinforce patient and caregiver education.
• Assist patients and caregivers with understanding medications, treatments, disease management, and safety precautions.
• Communicate changes in patient condition to the supervising RN and physician as appropriate.
• Follow the established plan of care and physician orders.
• Coordinate with RNs, physicians, therapists, HHAs, and other members of the care team.
• Complete accurate and timely visit documentation.
• Assist with follow-up visits, recertification-related care, and other skilled nursing services within scope of practice.
• Maintain compliance with agency policies, Texas nursing requirements, and applicable Medicare/home health regulations.
PRN Nurse Responsibilities
PRN nurses will provide coverage based on patient needs and availability, including:
• Routine skilled nursing visits.
• Start-of-care visits when qualified and assigned.
• Follow-up and supervisory visits.
• Wound care and medication management.
• Recertification and discharge visits as assigned and within scope.
• Resumption-of-care visits when qualified and assigned.
• Urgent or same-day patient visits when available.
• Weekend and holiday coverage as needed.
• Communicating patient changes promptly to the clinical team.
• Completing documentation within agency-required timelines.
What We Are Looking ForRN Requirements
• Current, unrestricted Texas RN license.
• Current CPR/BLS certification.
• Home health experience preferred.
• OASIS experience preferred.
• Knowledge of Medicare home health requirements preferred.
• Strong assessment and clinical documentation skills.
• Reliable transportation and willingness to travel within assigned territory.
• Ability to work independently in patients' homes.
• Strong communication and organizational skills.
LVN Requirements
• Current, unrestricted Texas LVN license.
• Current CPR/BLS certification.
• Home health experience preferred but not required for the right candidate.
• Strong clinical and documentation skills.
• Reliable transportation.
• Ability to travel within assigned territory.
