Registered Nurse – Home Health

We are seeking a dedicated Registered Nurse (RN) to provide skilled home health care throughout the community. This role offers a consistent Monday–Friday schedule with
no weekend or on-call requirements.





Position Details


- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Visits: 5 visits per day
- License: Active Wisconsin RN license required
- Mileage: $0.43/mile
- Coverage Areas: Fond du Lac, Dodge, Columbia, and Winnebago Counties
- Additional Locations: Brookfield, Jackson, Waterford, and Sheboygan






Responsibilities


- Perform comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing services and monitor patient progress.
- Provide wound care and manage complex medical needs.
- Coordinate care with physicians and other healthcare professionals.
- Complete accurate clinical documentation and OASIS assessments.
- Educate patients and caregivers on disease management and treatment plans.
- Identify changes in patient condition and communicate appropriately with providers.






Required Qualifications


- Active Wisconsin RN license
- Wound care experience
- Current CPR/BLS certification
- Valid driver’s license, reliable vehicle, and valid auto insurance






Preferred Skills


- OASIS assessment experience
- Home Care Home Base (HCHB) highly desired
- MatrixCare or other Electronic Medical Record (EMR) experience accepted
- Experience with complex wound care and home health services
