Hematology/Oncology Nurse Navigator - Care Continuum Lead

Beth Israel Lahey Health is seeking a Hematology/Oncology Nurse Navigator to be the first contact for incoming referrals and to coordinate patient access from initial contact through diagnosis, treatment and follow-up.

This role supports telephone triage, symptom management, and same-day clinic visits to avoid ER visits and hospital admissions. It also follows discharged patients to prevent readmission and improve continuity of care.
