Description

As a Hematology Oncology Nurse Navigator, you will play a crucial role in delivering exceptional patient care by coordinating treatment regimens and providing essential support for individuals diagnosed with cancer. Your expertise will guide patients and their families through a complex healthcare landscape, ensuring they have access to necessary services and programs that promote overall wellness.

Your personalized, compassionate care will address the unique needs of each patient, emphasizing not only their physical health but also their emotional and spiritual well-being. In this role, you will participate in health screenings, deliver vital patient education, and facilitate appropriate referrals to optimize patient outcomes.

You will be responsible for managing treatment regimens, tracking patient data related to contacts and referrals, and fostering meaningful connections with patients, family members, healthcare professionals, and community resources.

$20,000 Sign-on Bonus is available for eligible external candidates and rehires who meet the eligibility criteria, supplemented by a comprehensive benefits and compensation package starting on your first day.

Required Qualifications:
• Bachelor's Degree in Nursing
• California Registered Nurse License upon hire
• National Provider BLS certification - American Heart Association (to be completed within 30 days of hire)
• Oncology Nurse Certification (OCN), Advanced Oncology Nurse Certification (AOCN), or relevant certification obtained within 1 year of hire
• Current ONS chemotherapy administration card upon hire
• Minimum of 5 years as a Registered Nurse in a healthcare setting
• At least 3 years of experience in oncology nursing

Preferred Qualifications:
• Master's Degree in Nursing
