Geriatric Pain & Palliative Care NP / PA | New Grads Welcome | Build the Practice | Up to $150/hr

A pain and palliative care practice launching in the Cincinnati area is hiring NPs and PAs to care for geriatric residents across skilled nursing facilities. You'll follow the same patients over time instead of a rotating panel, managing their pain medications, including controlled substances, with an MD on site. This is a ground-floor role: you'll help shape the practice's clinical standards in a brand-new Ohio market. The practice is looking first for providers with pain management, ortho, physiatry, SNF rounding, or hospice experience. New graduates are considered with RN experience in ortho, pain management, SNF, or high-volume ER. Malpractice with tail coverage is included.

The role pays per visit, so your earnings grow with your productivity. You earn $10 per completed visit, with a guaranteed $150 minimum at each facility you see patients at in a day. Visits are focused pain management follow-ups of about 10 minutes, not full assessments. Most providers see 40 to 50 patients a day, or around $400 to $500 daily, and more on days spread across smaller facilities.

Key Responsibilities
• Conduct 40 to 50 focused pain management follow-ups per day across partner facilities
• Build individualized plans combining medication management, non-pharmacological approaches, and interventional referrals
• Prescribe and monitor pain medications independently, including controlled substances, per Ohio state and federal regulations
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and other interventions
• Educate patients and families on pain control strategies
• Track outcomes through consistent follow-up and refine treatment plans
• Document in TEBRA and PointClickCare
• Build strong relationships with staff and leadership at each facility

Qualifications
• Active, unrestricted Ohio NP or PA license, or eligible to obtain
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
• Active DEA registration, or able to obtain one before start
• Comfortable with 40 to 50 focused visits per day
• Comfortable prescribing independently, including controlled substances
• Fully onsite; comfortable traveling between Cincinnati area facilities
• Preferred: experience in pain management, ortho, physiatry, SNF rounding, or hospice with controlled-substance prescribing
• New grads considered with RN experience in ortho, pain management, SNF, or high-volume ER (county or trauma ER a strong plus)
• TEBRA or PointClickCare experience a plus

Schedule & Compensation
• $10 per completed visit | ~$400–$500/day at typical volume
• $150 guaranteed minimum per facility per day (a floor, not an extra payment)
