Evening Licensed Practical Nurse

Responsibilities
• Deliver compassionate evening nursing care to patients across hospice, home care, and long-term care settings.
• Administer medications and treatments following provider orders and organizational protocols.
• Monitor, document, and communicate patients' conditions, vital signs, and responses to treatment.
• Provide vital education and emotional support to patients and their families, ensuring effective care coordination.
• Work collaboratively with RNs, physicians, and interprofessional team members to implement care plans.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Adhere to infection control, safety, and quality assurance protocols at all times.
• Engage in ongoing training, team huddles, and performance improvement initiatives.
