Liberty Health is looking for a dedicated and empathetic Hospice Registered Nurse to provide exceptional, person-centered end-of-life care in both home and facility environments. As a Hospice RN, you will conduct thorough assessments, craft and adjust personalized care plans, administer medications and treatments, and expertly manage symptoms and pain. This role entails close coordination with physicians, social workers, and other professionals to offer both emotional and clinical support to patients and their families. We emphasize accurate documentation in the EMR, educating caregivers on disease processes and effective care techniques, and participating in on-call rotations and quality improvement initiatives.

Key Responsibilities
• Deliver specialized hospice nursing care in home and facility settings
• Conduct comprehensive assessments and ongoing patient evaluations
• Develop, implement, and regularly update individualized care plans
• Utilize evidence-based interventions for pain and symptom management
• Safely and accurately administer medications and treatments
• Provide education to patients and families regarding disease processes and caregiving techniques
• Collaborate with physicians and interdisciplinary team members for coordinated care
• Document all interactions and care provided in the EMR promptly
• Engage in on-call rotations, crisis intervention, and patient visits when necessary
• Contribute to quality improvement, compliance efforts, and best-practice initiatives
