Liberty Health is looking for a dedicated and compassionate Hospice Registered Nurse to deliver high-quality, person-centered end-of-life care in patients' homes and various facilities. In this fulfilling role, you will conduct comprehensive assessments, develop and adapt care plans, manage symptoms, and coordinate interdisciplinary services to ensure every patient receives empathetic and effective care. Your responsibilities will include educating patients and their families, maintaining comfort and dignity, accurately documenting clinical information, and collaborating with physicians, social workers, and spiritual care providers. You will be part of a team that supports evidence-based, outcomes-focused hospice care while improving quality across various populations.

Key Responsibilities:
• Deliver hospice nursing care centered on comfort and symptom alleviation.
• Conduct thorough patient assessments and create personalized care plans.
• Administer medications, treatments, and interventions following hospice protocols.
• Educate patients and families on the disease process, medications, and end-of-life care options.
• Collaborate with physicians, social workers, chaplains, and interdisciplinary team members to coordinate care.
• Monitor changes in patients' conditions and adjust care plans as necessary.
• Ensure all assessments, interventions, and outcomes are documented accurately and in a timely manner.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
• Provide emotional and psychosocial support to patients and families.
• Ensure all care provided meets regulatory, safety, and ethical standards.
