In this pivotal role, you will assess patient needs, develop customized care plans, and deliver essential medications and treatments, all while managing symptoms to enhance comfort and quality of life. Key Responsibilities
• Conduct thorough patient assessments and continuous evaluations in diverse settings
• Create, implement, and refine personalized hospice care plans
• Administer medications and treatments, including IV therapies, in accordance with physician orders
• Effectively manage pain and complex symptoms to ensure maximum patient comfort
• Educate patients and families about disease progression, treatment options, and comfort measures
• Maintain accurate documentation of care, assessments, and communications in the Electronic Health Record (EHR)
• Collaborate with physicians, social workers, chaplains, and aides as a vital member of the interdisciplinary team
• Participate in on-call rotation and respond promptly to urgent patient needs
• Advocate for patient wishes, dignity, and end-of-life goals with compassion and respect
• Ensure adherence to hospice regulations, safety, and infection control standards

Required Skills
