Location: Northern
Lifted Hospice in Texas is seeking a Clinical Navigator to blend nursing expertise with compassionate guidance, assess eligibility, educate patients and families, coordinate admissions, and work with physicians and the Medical Director to ensure timely, seamless care.
You will also conduct daily GIP visits, maintain compliant documentation in EMR, build relationships with referral partners, and uphold Medicare/CHAP regulations while delivering patient-centered service.
