Compassionate After-Hours Hospice LPN – Make an Impact

Catholic Community Health is seeking an On-Call Hospice LPN to support patients and families across the Kansas City metro and surrounding communities from Lenexa, Kansas. This role focuses on after-hours nursing support, triage, symptom management, and coordinated crisis care.

You will document care in MatrixCare EMR, collaborate with physicians and the hospice team, and provide compassionate, patient-centered care during critical moments.
