Job Description
Role Overview: This pharmacist role involves providing safe, effective, and cost-efficient medication therapy to patients, developing and monitoring evidence-based care plans with a focus on patient outcomes and quality of life.
Key Responsibilities:
• Delivering pharmaceutical care in assigned patient care areas.
• Interpreting and clarifying physicians' medication orders.
• Reviewing medication profiles for safety, effectiveness, and interactions.
• Recommending formulary alternatives and advising healthcare professionals on drug therapy issues.
• Monitoring laboratory results and providing patient discharge medication consultations.
• Collaborating with medical staff, making rounds, and calculating drug dosages.
• Preparing compounded and sterile medications using aseptic techniques.
• Supervising pharmacy staff, technicians, students, and interns.
• Coordinating pharmacy activities with healthcare teams and maintaining departmental policies.
Qualifications & Skills:
• Bachelor's Degree in Pharmacy required; Doctoral Degree preferred.
• Strong knowledge of pharmacotherapy, medication safety, and clinical monitoring.
• Excellent communication, collaboration, and supervisory skills.
Additional Duties: Ensuring compliance with organizational policies, managing resources efficiently, and participating in departmental policy reviews.
This role is performed in a clinical environment, emphasizing patient safety, quality care, and professional collaboration.
