General Summary

Under the supervision, coordinate of clinical services for the radiation oncology patients, including related procedures services with the primary focus in improving experience and assisting the clinical team for the efficient and effective delivery of patient care.

Principal Responsibilities And Tasks

• Working with the Radiation Oncology clinical teams as well as referring physicians and patient to coordinate services for the radiation oncology department and related procedure services
• Obtain and oversee the distribution of patient clinical information with the clinical team to coordinate the appropriate patient care
• Attend and participate in associated multi-disciplinary patient related conferences, huddles and tumor boards
• Working with the radiation oncology clinical team to maintain an active, pending and completed patient list
• Manage internal procedure related communication
• Working with other involved departments arranging all consultations, follow ups, test, procedures and all treatment related appointments
• Act as a liaison for patient and families for all procedure related
• Participate in community social outreach when appropriate
• Communicate regularly with the patient and family and the involved clinical team regarding all aspects of care
• Ensure that all patient flow issues; physician initial evaluation visit, procedure(s), follow up visits are managed effectively and efficiently
• Assist physician and clinical team with procedures
• Secure/ manage medical supplies and equipment for procedures
• Order supplies if required for procedures
• Coordinate post procedure equipment and procedure room cleaning
• Ensure that all patient receive a post treatment wellness check in the form of a telephone call or an audio-visual call
• Working along with departments social work team to help minimize or address social barriers to care
• Communicate with referring providers after completion of treatment to coordinate follow up care
• Ensure treatment and other relevant medical records be shared with out-network referring physicians
• Working principally with the Radiation Oncology research team to support related research data collection and reporting if required

Work Experience

Education and Experience

• High School Diploma required
• Minimum of three (3) years clinical experience required. Oncology-specific experience is strongly preferred
• Current license in related health care field (Nursing, Medical Assistant Medical)

Knowledge, Skills And Abilities

• Clinical and professional knowledge of current issues relevant to care delivery stems for specific patient populations. Additional knowledge in the areas of professional development, group process, change management and effective leadership is also required.
• Ability to work autonomously in the management of projects and programs is required. Initiative and problem-solving skills are needed.
• Ability to develop collaborative programs and projects with other disciplines is required. Must be able to contribute to team effectiveness, build relationships and facilitate improvements.
• General knowledge of PC and database management software packages is required.
• Highly effective verbal and written skills are required to work with all levels of hospital personnel, administrators and clinical staff.
• Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient populations(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
