POSITION OVERVIEW
DHI365 is seeking a Certified Registered Nurse Practitioner (CRNP) to join our growing clinical team.
This position is fully telehealth-based and offers the opportunity to work closely with physicians, care managers, and clinical support staff to provide high-quality virtual care to patients enrolled in our care management programs.
The ideal candidate is clinically strong, patient-focused, comfortable working in a technology-driven environment, and experienced in managing patients remotely through telehealth and care coordination programs.
KEY RESPONSIBILITIES
Telehealth Patient Care
• Conduct virtual patient evaluations, follow-up visits, and ongoing telehealth consultations
• Assess, diagnose, treat, and manage patient conditions within scope of practice and applicable regulations
• Develop and update patient care plans based on clinical findings and ongoing monitoring
• Provide patient education regarding disease management, medications, treatment plans, and follow-up care
Care Management Programs
• Evaluate patient eligibility and clinical need for Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), Behavioral Health Integration (BHI), and other care coordination programs
• Collaborate with care managers and clinical staff to support patient engagement and successful program participation
• Review patient progress and recommend interventions when appropriate
Clinical Documentation & Compliance
• Maintain accurate, timely, and complete clinical documentation within the electronic health record system
• Document patient encounters, care plans, clinical interventions, and follow-up recommendations
• Ensure compliance with all applicable healthcare regulations, documentation standards, and privacy requirements, including HIPAA
Clinical Collaboration
• Work closely with supervising physicians and interdisciplinary care teams to support patient care and care coordination initiatives
• Escalate complex clinical concerns and patient care issues when appropriate
• Participate in clinical meetings, quality improvement initiatives, and protocol development
QUALIFICATIONS
• Current Certified Registered Nurse Practitioner (CRNP) license in good standing
• Graduate of an accredited Nurse Practitioner program
• Ability to assess, diagnose, treat, and manage patients within scope of practice
• Strong clinical judgment and decision-making skills
• Excellent communication and patient education skills
• Strong documentation and organizational skills
• Ability to work independently while collaborating effectively with physicians and care teams
• Working knowledge of HIPAA, healthcare compliance requirements, and telehealth best practices
PREFERRED EXPERIENCE
• Previous telehealth experience strongly preferred
• Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), or Behavioral Health Integration (BHI)
• Experience working with Medicare populations and care coordination programs
• Familiarity with electronic health record systems and virtual care platforms
• Experience supporting patients with chronic conditions and long-term care needs
SCHEDULE
• Full-time, Part-time or Contract opportunities available

COMPENSATION & BENEFITS
• $50.00 – $70.00 per hour (based on experience, credentials and payer enrollment status)
