Case Manager

Role Description The Case Manager is a full-time, on-site role based in Philadelphia, PA, responsible for providing direct support to young people experiencing or at risk of homelessness. Day-to-day responsibilities include conducting intake assessments, developing individualized service plans, and coordinating housing, healthcare, education, and employment resources. The Case Manager will meet regularly with youth to monitor progress, advocate for their needs, and adjust plans as circumstances change. This role also involves maintaining accurate documentation, collaborating with internal teams and community partners, and ensuring services are delivered in a trauma-informed, strengths-based, and culturally responsive manner. The Case Manager will help youth build life skills, access benefits, and connect with supportive networks to promote long-term stability and independence.





Qualifications


- Experience in case management, social work, counseling, or a related field, preferably serving youth or individuals facing homelessness.
- Strong skills in assessment, care planning, and coordination of services, including housing, healthcare, education, and employment supports.
- Effective communication and interpersonal skills, with the ability to build trust, maintain professional boundaries, and work with diverse populations.
- Organizational and documentation skills, including accurate record-keeping, time management, and familiarity with basic computer applications.
- Knowledge of community resources, public benefits, and systems that impact youth experiencing homelessness is highly beneficial.
- Ability to work collaboratively within a multidisciplinary team and with external partners, and to respond calmly to crises and changing needs.
- Commitment to trauma-informed, youth-centered, and non-judgmental practice; ability to uphold confidentiality and ethical standards.
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field preferred; equivalent combination of education and relevant experience considered.
