Job Description
The Care Continuum Partner - Medical Assistant role supports patients and clinical teams by ensuring smooth care delivery both within physical and virtual settings.
Responsibilities include assisting with pre-visit planning, managing care gap closures, referrals, and medication authorizations, and conducting patient outreach through various communication platforms.
It involves facilitating virtual appointments, providing patient education, managing prior authorizations, and supporting annual well visits.
The position acts as a liaison for patient portal and medical record technology, offering virtual care assistance.
Qualifications include a Medical Assistant diploma or specialized certification, with at least 2 years of patient interaction experience and 3 years in a complex healthcare environment.
Preferred candidates possess an Associate’s Degree, knowledge of electronic medical records, medical terminology, and relevant certifications such as CCMA.
Skills required include attention to detail, excellent communication, customer service, adaptability, and critical thinking.
Physical demands involve lifting, sitting, standing, and performing activities like kneeling or CPR as needed.
