Breast Oncology Care Coordinator - Fox Chase Cancer Center

The Breast Oncology Care Coordinator is a multi-site role that works in conjunction with the Oncology Nurse Navigator(s) and the breast service line team to ensure access for individuals with suspected or confirmed breast cancer to quality care that is timely, coordinated and patient-centered.

Education
Bachelor's Degree Healthcare or related field Required
Other within 6 months of hire completion of competency-based training and education in patient navigation from a qualifying training program Required

Experience
2 years experience in a healthcare setting or equivalent preferably in oncology Required
General Experience and knowledge of basic clinical concepts Required
General Experience and knowledge of EPIC systems Required
General Experience and knowledge of complex patient issues and social determinants of health Required
General Experience and knowledge of patient outreach methods and strategies Preferred
General Experience with medical terminology Preferred
General Experience working as a patient navigator/community health worker Preferred
