As a Breast Care Navigator, you'll serve as a trusted guide, advocate, and resource for patients throughout their breast health journey, helping coordinate care, remove barriers, and support positive clinical outcomes from diagnosis through survivorship.
What You'll Do
Initiate communication with patients following abnormal or suspicious breast imaging findings.
Facilitate communication among patients, physicians, providers, and interdisciplinary care teams.
Coordinate imaging appointments, biopsies, referrals, consultations, and follow-up services to support timely diagnosis and treatment.
Follow patients from initial abnormal imaging through diagnostic resolution, treatment, and survivorship planning.
Provide patient and family education regarding diagnoses, treatment options, recommended follow-up care, and available support resources in collaboration with the physician care plan.
Identify and address barriers to care, including scheduling challenges, transportation concerns, financial obstacles, and access to support services.
Monitor patient progress throughout the continuum of care and provide ongoing guidance and support.
Participate in multidisciplinary breast conferences, quality improvement initiatives, and community outreach activities.
Track patient interventions, outcomes, and performance measures to support program development and continuous improvement.
Serve as a central point of contact among radiology, breast surgery, pathology, medical oncology, radiation oncology, rehabilitation services, genetics, and supportive care resources.
What You'll Bring
Required Qualifications
Current Missouri Registered Nurse (RN) license in good standing or current Registered Radiologic Technologist with Mammography experience and credentials.
Strong communication and interpersonal skills.
Excellent organizational and care coordination abilities.
Commitment to patient-centered care and advocacy.
Ability to build collaborative relationships with patients, families, providers, and clinical teams.
Preferred Qualities
Breast imaging, mammography, oncology, or breast health experience.
Patient navigation or care coordination experience.
Knowledge of breast cancer screening, diagnosis, treatment, and survivorship resources.
Experience in patient education and quality improvement initiatives.

Frequent walking, standing, and movement throughout clinical areas.
Regular communication with patients, providers, and healthcare teams.
Computer-based documentation and care coordination activities.
Education, Certifications & Licensure
Current Missouri Registered Nurse license in good standing or current Registered Radiologic Technologist with Mammography credentials.
Current Basic Life Support (BLS) certification required.
Three or more years of relevant healthcare experience preferred.
